Business-contact privacy notice

A plain-language explanation of the limited information used to review a U.S. Urgentis Doctor pilot request.

No patient information belongs in this form. A medical practice remains responsible for its HIPAA Notice of Privacy Practices. Urgentis will not process PHI for a practice until the applicable contract, BAA and technical gates are in place.

1. Scope of this notice

This notice applies to U.S. website visitors, founding-practice applicants, prospects and customer business contacts. It covers the information submitted to start a business conversation with Urgentis Doctor.

Do not submit patient names, symptoms, appointment details, medical record numbers or other protected health information through the founding-practice form. This notice is not a medical practice's HIPAA Notice of Privacy Practices.

2. Who operates Urgentis

Urgentis is operated by Pixel Company, a Belgian simple partnership registered under enterprise number 1016.324.626 and VAT number BE1016324626, at Rue de l'Éléphant 7 box 6, 1080 Molenbeek-Saint-Jean, Belgium.

3. Information we collect

  • Your name, professional role, business email and optional business phone number.
  • Practice name, U.S. state, number of providers, EHR or scheduling system and the administrative workflow you want to improve.
  • Basic security and abuse-prevention information, such as IP address, request time and technical logs.
  • Commercial, support and contract records if the conversation progresses beyond the application.

4. Why we use it

  • To receive, review and respond to your pilot request.
  • To understand practice workflows and determine whether a controlled pilot is appropriate.
  • To protect the application form and service against abuse, fraud and security incidents.
  • To maintain necessary business records and defend legal rights.
  • To send relevant Urgentis business communications; you may ask us to stop optional outreach at any time.

5. Sharing, sale and advertising

We may use contracted providers for hosting, databases, email, security, customer support and professional advice, only to support the purposes described above and subject to appropriate terms.

Pixel Company does not sell the business-contact information submitted through this form and does not share it for cross-context behavioral advertising. We do not place advertising trackers on authenticated or patient-facing product pages.

6. International processing

Pixel Company is established in Belgium, so business-contact information may be accessed or processed in Belgium and other disclosed service locations. We apply safeguards required by applicable law to cross-border processing.

A future U.S. patient-data service will use a separately approved U.S. data plane, signed customer contract and Business Associate Agreement. This pre-launch form does not activate that service.

7. Retention

We retain business-contact information only for as long as reasonably necessary to review and respond to the request, manage a resulting commercial relationship, protect the service, comply with legal obligations and resolve claims. The period depends on whether the request becomes an active customer relationship. We then delete or anonymize the information when it is no longer needed, subject to required legal records and defensible security backups.

8. Your choices

You may ask for access to, correction of or deletion of your applicable business-contact information, or object to optional marketing, by emailing contact@urgentis.be. We may need to verify your identity before acting on a request and may retain information where the law permits or requires it.

These choices are offered for this pre-launch contact process without limiting any additional rights that may apply under your state or other applicable law. Patient requests concerning a medical practice's records should be directed to that practice.
